How to Complete Your NPDB Self-Query
Your NPDB Self-Query must be submitted with your onboarding form. It is also required every 180 days, so you may receive reminders from Credentialing to complete it again.

Understanding ID.me + NPDB
When you order an NPDB Self-Query, you’ll be asked to sign in with ID.me.
ID.me is only the login system NPDB uses to verify your identity.
ID.me and NPDB are separate. Creating or signing in to ID.me does not complete your Self-Query.
After verifying with ID.me, you must return to NPDB and finish ordering your Self-Query.
Think of ID.me like “Sign in with Google” — it’s just the login method. The actual Self-Query is ordered through NPDB.
Step 1: Access NPDB
Go to the NPDB Self-Query page.
Click Sign In → Sign In to a Self-Query Order.
Click Sign in with ID.me.

Step 2: Sign in with ID.me
Sign in (or create an ID.me account).
Complete identity verification as prompted.
Step 3: Start a New Order
Return to NPDB.
Click Start a New Order.
Select Personal as the search type.
Step 4: Enter Details
Provide your personal, address, and professional info.
Include your state license (if applicable) or your CDR number.
Enter your NPI.
Step 5: Pay Fee
Pay the $3 fee (reimbursable via Ramp – see reimbursement guide).
Step 6: Submit + Download + Upload
Submit your order and monitor the status.
Once available, click Show Me to download your report.
Attach and upload your NPDB report in the Documents section of your onboarding form.
FAQs
Do I need to create an ID.me account?
Yes. Everyone logs in through ID.me. If you don’t have an account, click “Sign in with ID.me” and follow the prompts to create one.
Am I reimbursed for the $3 fee?
Yes. Ramp reimburses the fee (attach your NPDB receipt).
Why did I get an email from HHS-XMS?
HHS-XMS (Health and Human Services External User Management System) is part of the secure login process. Authorize it when prompted — it’s normal and required.
