What to Expect With Your Verifiable Onboarding Form

The Verifiable onboarding form is a short online form you’ll complete through a Salesforce portal.

As part of filling it out, make sure your CAQH is set up and that you’ve ordered your NPDB Self-Query.

You’ll receive an email from credentialing@usenourish.com with the subject line “Nourish – Verifiable Account Creation.”

Click the link in the email to create your Verifiable account.

You’ll be redirected to a Salesforce login page — this is correct.

If your link has expired, click Forgot my password and use your username provided in the email to reset your password and log in.

After logging in, you’ll see your online onboarding form.

Complete all sections of the form with your basic personal information.

Note: You’ll need your NPI to move forward.

If you don’t have one yet, follow the NPI guide before continuing.

Enter your CAQH username, password, and CAQH ID.

Then confirm your CAQH permission preferences.

Note: This is where you can choose to have Nourish manage your CAQH for you.

Upload your:

• Resume

• NPDB Self-Query

• Signature file

Once everything is filled out and uploaded, click Submit.

Our team will review your submission and follow up if anything else is needed.
