Which Sections Do I Need to Complete in CAQH?

You’re only responsible for completing a few sections of your CAQH profile. Our team will add all Nourish details afterward and handle attestation.

Instructions

1. Go to CAQH ProView.

2. Click Profile Data to view all sections.

3. Complete the following sections yourself:

   • Personal Information

   • Professional IDs

   • Education & Professional Training

   • Specialties

   • Employment Information

   • Disclosure

4. Once you’ve filled out those sections, click Review & Attest.

Note: If your profile is being set up for the first time, it won’t let you attest until all sections are added. That’s fine — the information you entered will save, and our team will attest once we’ve added the remaining details.
