Why This Information is Needed
Your CAQH login is required as part of the credentialing process.
Our credentialing verification organization requires a valid CAQH login on file to confirm your profile is active, accurate, and up to date.
Without a valid login on file, we won’t be able to complete credentialing with all payers.
Providing Your Login ≠ Permission to Update your Profile
ℹ️ Permission to make updates to your profile is a separate decision that you control on the onboarding form.
Your Choice is Respected
If you choose not to allow updates:
We will not update your CAQH profile
You will manage your CAQH profile yourself
Your selection is documented and respected, and you can change it at any time.
